Following are the questions which will have to be answered by all persons between the ages of 21 and 30 inclusive who will register on the 5th of June for the conscriptive draft. Are you (1) a natural born citizen, (3) a naturalized citizen, (3) an alien, (4) or have you de clared your intention (specify which) ? 1 5. Where were you born, town, state, nation. 6. If not a citizen, of what coun try are you a citizen or subject? 7. What is your present trade, occupation, or office? 8. By whom employed? Where employed? 9. Have you a father, mother. wife, child under -12, or a sister or brother under 12, solely de- dependent on you for support (specify which) ? 10. Married or single (which) ? Race (specify which) ? 11. What military service have you had? Rank; branch; years; Nation or siate 12. Do you claim exemption from draft (specify grounds)?
